Request an appointment with a Nurse or Healthcare Assistant

Tell us when you want your appointment and we will reply to you with a time. Once we receive this form we will process it within 1 working day.

Please do not use this form to request an appointment with a Doctor or Nurse Practitioner, please use the Get Help from a Doctor or Practitioner form.

Please be aware that any replies from the surgery may appear in your junk or spam inbox.

Blood Tests

Due to National Shortage for blood bottles, we have been advised to book urgent blood tests only.

We will therefore be postponing some blood test appointments until we are advised otherwise. This will not affect you speaking to a clinician or receiving the help you need, please contact us if needed.

Please continue to order your repeat prescriptions as normal. We do advise that if you are able to book an appointment at the hospital for a blood test following the request of your consultant, please book using this number 01603 286921.

We will be reviewing this issue regularly and hope to return to normal soon.
