Subject access request (SAR)

Use this form for any subject access request, this is to gain information from your clinical records.

This request will be passed to our admin team and can take up to 30 days to complete. Depending on the nature of the request, we may require a further extension of 30 days.

You can use this service if you:

• are registered at the surgery

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• information you require from your medical record
• you may be contacted regarding this request
