Temporary patient registration

Use this service to register as a temporary resident with Watton Medical Practice for up to three months.

You can use this service if you:

• want to temporarily register at the surgery

This will enable you to be on the practice list whilst still remaining a patient at your home GP surgery.

You should only register with us as a temporary patient if you need medical attention whilst you are away from your home GP.

You cannot register as a temporary patient if you are registered with a GP surgery in our local area.

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• your NHS number
• information about your health
• other personal details and demographics
