The Role of the Medication Team

The Medication Team plays a crucial role in ensuring that all patient medication requests are processed safely, accurately, and efficiently—regardless of how the request is received. This includes requests submitted via online services, email, the practice website, or in person at the surgery.

Core Responsibilities

  • Process all medication requests from start to finish, ensuring each is handled promptly, accurately, and in line with practice protocols.
  • Provide direct patient support for medication-related queries through:
    • Medication Request Line – open Monday to Wednesday, 10:00–12:00
    • Medication Front Desk Support – available:
      • Tuesday: 14:30–16:30
      • Wednesday: 13:00–15:00
      • Thursday: 14:00–16:00
      • Friday: 13:00–15:00
  • Ensure patients have up-to-date medication reviews to support the safe continuation of repeat prescriptions
  • Process Personally Administered Prescriptions (PPA) issued by clinicians and nurses, and complete monthly claims
  • Review and action medication-related correspondence, with all letters checked by two team members for safety and accuracy
  • Monitor shared care medications, ensuring patients complete required blood tests, attend hospital appointments, and meet all ongoing monitoring requirements
  • Carry out reporting and internal audits to confirm accurate dosing and compliance with clinical guidelines
  • Support patient safety and practice efficiency, playing a key role in medication management behind the scenes
